Sudden onset
of sever morning headache, visual impairment follow by loss of
consciousness with focal fits. During
examination he was deep coma with bilateral pinpoint pupil with
right sided weakness.
Brain CT scan show extensive subdural, intraventricular
hemorrhage with obstructed hydrocephaly.
Fill doses of
factor VII with urgent surgery was done for fixation the
external ventricular drainage system to decrease intracranial
pressure and shifting obstructed cerebrospinal fluid.
After surgery he is improve conscious level but became more
irritable. After 3 days
cerebrospinal fluid drainage
become more clear and
Brain CT scan show no hydrocephaly so removal of the external
device was done. Fifth postoperative day the patient more stable
with good power, eating and walking normally.
I send the patient for further evaluation to excluded underlying
aneurysm.
